Please read these terms and conditions before using Quality Life Enablement.
Your access to and use of the Service is conditioned on your acceptance of and compliance with these Terms. These Terms apply to users and others who access or use the Service.
By accessing or using the Service you agree to be bound by these Terms. If you disagree with any part of the terms, then you should not access the Service.
Quality Life Enablement’s Mission
Quality Life Enablement’s mission is to offer high-quality Occupational Therapy. Quality Life Enablement offers 2 services to users: Occupational Therapy and Enablement Support. Both services aim to improve people’s quality of life, health and wellbeing through enabling people to engage in activities (occupations) that are important to them.
Quality Life Enablement’s Services
The Occupational Therapy service includes Occupational Therapy Assessments that take place in the client’s home however could also occur in another community setting, depending on client’s needs, upon agreement with therapist. Following an assessment, follow up rehabilitation can be arranged. These visits can take place both in the home and out in a community setting, depending on the client’s needs.
Enablement Support Service is for adults of all ages. It involves 1-1 support to engage in activities that are agreed by both the client and the therapist. This can be either in the home or out in the community. The therapist has the right to decline supporting with certain activities if it puts the health and safety of the client, therapist or public at risk.
As Quality Life Enablement provides 2 services, we reserve the right to ensure the client is receiving the correct service for their needs.
Fees
Occupational Therapy Assessments are charged at £65 for a 60-minute assessment. Every 15 minutes after the first 60 minutes is charged at an additional £15. Charges for written reports and recommendations will vary depending on the level of detail required.
Rehabilitation sessions are charged at £50 per hour with every additional 15 minutes charged at an additional £12.50.
The hourly charge for Enablement Support starts from £30 per hour. These fees can be variable depending on a multitude of factors including the individual’s level of support needs and the activities that will be undertaken during the visit. The charge will always be agreed with Catherine Gough, in advance to any sessions to ensure transparency of fees.
Additional charges
The therapist’s travel time to and from the assessment/rehabilitation site may be charged depending on location from therapist’s base however this will be agreed before any visits.
When receiving support from the Enablement Support Service £0.45p per mile is chargeable for any outings during the visit. Clients are also responsible for all additional costs associated with outings including refreshments or other travel methods to venues.
Payment of Fees
Payment can be made up front using a debit or credit card immediately after the visit using a card reader. A receipt can then be sent via email. Or payment can be made via bank transfer on receiving an invoice from Catherine Gough. This payment must be received within 14 days of receiving the invoice.
Services may be withdrawn at any time if payment remains outstanding.
Cancellation Policy
There is a 50% charge for appointments that are missed or cancelled less than 48 hours before the scheduled date/time. This is because cancellation within this time will result in gaps in scheduling and loss of earnings for therapist. I will endeavour to give you as much advance notice as possible in terms of holidays or of any need to cancel a future appointment.
The Therapist
Is registered with the Royal College of Occupational Therapists/British Association of Occupational Therapists and abides by the Code of Ethics.
Holds BAOT (British Association of Occupational Therapists) Malpractice and Professional Liability and Public and Products’ Liability Insurance.
Engages in continuous professional development.
Are registered with the Health and Care Professions Council (HCPC) http://www.hcpc-uk.org/check-the-register/.
Complaints Procedure
Quality Life Enablement is committed to providing a high level of service to all clients. If you do no recieve satisfaction we need to hear from you. This will help us to improve our standards.

Keeping Records
Keeping notes is an integral part of the practice of all Occupational Therapists and is a requirement as part of the duty of care to individuals we work with. Care records will be kept following each session in line the Health and Care Professions Council requirements, the College of Occupational Therapist Code of Ethics and Professional Conduct and insurance policy requirements.
All information and Care Records will be stored in accordance with UK General Data Protection Regulations (2018).
Data Protection
Quality Life Enablement is committed to ensuring your data is protected. Should you provide information by which you can be identified when working with Quality Life Enablement, you can be assured that it will only be used in accordance with this privacy statement.
The type of personal information we collect:
We may require basic information which identifies you as an individual (“Personal Information”) such as your name, email address and phone number. We will only use such Personal Information for the purposes of providing information which you have requested, or for other purposes set out in these Terms.
We may collect the following information:
Name
Company or organisation
Contact information, including email address and telephone number(s)
Demographic information such as postcode, preferences and interests
Information pertinent to fulfilling our services on an individual’s or organisation’s behalf.
How we get the personal information and why we have it:
Most of the personal information we process is provided to us directly by you.
We require this information for the purpose of:
Professional record keeping
Recording the initial assessment
Provision of Occupational Therapy services
Sharing your information with relevant third-parties when necessary.
How we store your personal information:
Client paper information is stored in a locked filling cabinet.
Customer electronic information is password-protected, has antivirus with firewall protection. The electronic device ie laptop is not a shared device and the primary use is for business.
We keep our internal records for a minimum of 7 years, in accordance with insurance policy requirements. We will then dispose of your information by deleting any electronic records we hold.
Who do we share your information with and how:
We may share information with third parties only when requested and with client consent. Information will only be shared with third parties without consent if it is justified in the public’s best interest or required by law.
Your Data Protection Rights:
Under data protection law, you have rights including:
Your right of access - You have the right to ask us for copies of your personal information.
Your right to correct - You have the right to ask us to rectify personal information you think is inaccurate. You also have the right to ask us to complete information you think is incomplete.
Your right to erase - You have the right to ask us to erase your personal information in certain circumstances.
Your right to restriction of processing - You have the right to ask us to restrict the processing of your personal information in certain circumstances.
Your right to object to processing - You have the right to object to the processing of your personal information in certain circumstances.
Your right to data portability - You have the right to ask that we transfer the personal information you gave us to another organisation, or to you, in certain circumstances.
Your right to object- You have the right to object to object to us using your personal data at any time.
