These Terms & Conditions explain how Quality Life Enablement provides Occupational Therapy services and what you can expect when working together. By booking or receiving services, you agree to these terms.
1. Mission
At Quality Life Enablement, my mission is to help older adults feel safer, stronger, and more confident in their everyday lives. I believe that everyone deserves to move through their home and community with confidence and a sense of control. Through personalised assessment and one‑to‑one rehabilitation, I work alongside each person to reduce falls risk, build confidence, and support meaningful daily activities. My approach is rooted in evidence‑based Occupational Therapy and always focused on what matters most to the individual and their family.
2. Services Provided
Quality Life Enablement offers:
Falls and Home Safety Assessments
Balance and Everyday Movement Support
Rehabilitation to rebuild daily living skills
An initial assessment must be completed before any follow‑up rehabilitation. Assessments usually take place in the client’s home but may occur in a community setting if appropriate and agreed.
Follow‑up sessions may take place at home or in the community, depending on the client’s needs.
3. Fees and Additional Charges
Session Fees:
Falls & Home Safety Assessment: £65 per hour
Additional time: £30 per extra 30 minutes
Follow‑up rehabilitation sessions: £60 per hour
Mileage:
Mileage is charged at £0.45 per mile after the first 5 miles of the return journey from the therapist’s base.
4. Payment of Fees
Payment can be made in one of the following ways:
Debit or credit card immediately after the visit (via secure card reader)
Bank transfer upon receiving an invoice
Invoices must be paid within 14 days. Services may be withdrawn if payment remains outstanding.
5. Cancellation Policy
Appointments cancelled with less than 48 hours’ notice will be charged at 50% of the session fee.
Missed appointments without notice will also be charged at 50%.
This policy is in place because late cancellations result in loss of income and limited availability for other clients.

I will always give as much notice as possible if I need to cancel or rearrange an appointment.
6. Professional Standards
Catherine Gough at Quality Life Enablement:
Is registered with the Health and Care Professions Council (HCPC)
Is a member of the Royal College of Occupational Therapists/British Association of Occupational Therapists
Holds Professional Liability and Public Liability Insurance
Engages in ongoing Continuing Professional Development

7. Complaints Procedure
I aim to provide a high‑quality, person‑centred service. If you are not satisfied, please contact:

Your feedback helps improve the service.
8. Record Keeping
Clinical notes are kept after each session in line with HCPC and RCOT requirements.
Records are stored securely in accordance with UK GDPR (2018).
Paper records are kept in a locked filing cabinet.
Electronic records are password‑protected and stored on secure devices.
Some records are stored using Zanda, a secure practice‑management system designed for healthcare data.
Records are kept for a minimum of 7 years, after which they are securely deleted.
9. Data Protection and Privacy
Quality Life Enablement is committed to protecting your personal information. A summary of how your data is used:

Information relevant to providing Occupational Therapy services

Professional record keeping
Completing assessments
Providing therapy services

Zanda software: secure, healthcare‑grade data protection
